Rayleigh and District PCN are a forward-thinking and friendly primary care network (PCN) looking for an experienced advanced nurse practitioner role to join our team. This role will be based within Audley Mills GP Surgery, 57 Eastwood Road, Rayleigh. We are looking for someone who can work full time hours Monday to Friday (9am – 5pm as an example) however other working arrangements will be considered.
This role will involve undertaking clinics, in a busy environment, but may also require the post-holder to make home visits in order to provide treatment to patients in their own homes.
