About the Role

Rayleigh and District PCN are a forward-thinking and friendly primary care network (PCN) looking for an experienced advanced nurse practitioner role to join our team. This role will be based within Audley Mills GP Surgery, 57 Eastwood Road, Rayleigh. We are looking for someone who can work full time hours Monday to Friday (9am – 5pm as an example) however other working arrangements will be considered.

This role will involve undertaking clinics, in a busy environment, but may also require the post-holder to make home visits in order to provide treatment to patients in their own homes.

About the Candidate

We are looking for someone who is organised and who thrives in a busy environment, where no two days are the same.

All applicants should:

• Be a registered nurse, holding a current registration with the NMC.
• Hold a recognised ANP qualification at master’s level.
• Hold an independent prescribing qualification and prescribe safe, effective, and appropriate medication.
• Be able to work with a high degree of autonomy.
• Be forward-thinking and will strive to develop innovative ideas and new ways of working, that address the health needs of the population, whilst motivating and encouraging others to do the same.
• Deliver a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth, theoretical knowledgebase.
• Assess, diagnose, and treat patients’ conditions, initiating referrals as appropriate.
• Support members of the multidisciplinary team by providing leadership, education, and training to a range of staff.
• Provide supervision to the PCN nurse associate.
