Our new Community Support Workers will join our multidisciplinary integrated teams in North & East Cornwall delivering reablement as part of a collaboration between community, acute and social care providers.

The reablement approach supports people to return to or remain in their homes. We are looking for candidates with the ability to motivate individuals in order to aid their recovery and maximise their independence and achieve their goals.

The post holder will be a valuable member of the community team to support the Ageing Well clinical pathway.

The post holder will be rostered to provide a 24 hour, 7 days a week service to community patients.

This post holder will be expected to work with patients in their usual place of care this may also include Residential homes and nursing homes.

Main duties of the job

  • The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting
  • The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital
  • The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes
  • The postholder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service.

Qualifications & Experience

Essential criteria
  • NVQ Level 3 qualification in care or equivalent experience
  • Evidence of experience in care setting
Desirable criteria
  • Additional relevant training in Healthcare
  • GCSE Maths & English or equivalent qualification

Skills and Aptitude

  • Good communication skills
  • Ability to work without direct supervision and to time restraints
  • Flexible in attitude and willing to support in other teams when required
  • Ability to plan own workload and organise required activities
