An exciting opportunity has arisen for a registered Occupational Therapist/Social Worker/or Registered Mental Health Nurse or equivalent professional to play a pivotal role in delivering this service, as well as planning, evaluating, and contributing to its development. We are looking for a professional who is driven, passionate and excited to be part of a strategically important service provision, in line with our programme of Community Mental Health Transformation which is aiming to transform and enhance the way we deliver our mental health services and aligned with the Trust's vision as 'One-Team'.
The service will employ an innovative model to provide out of area service delivery, gatekeeping for S117 after care applications, out of area placements/independent hospitals and individual funding requests for specialist complex mental health care for people with severe mental health issues.
