Sheffield City Council has been successful to be a staying close pilot area and has received external funding to strengthen the offer to care leavers age 16+ leaving residential care and transitioning to independent living. This has provided an exciting opportunity to join a multi-Agency Team and we are looking to appoint a full-time Family Group Conferencing Activities coordinator fixed term for up to 2 years who will work on this project as an activities co-ordinator with a multi-agency team in order to:

  • Build early relationships and prepare young people for independence
  • Provide seamless support to the young person before and after moving into their Staying Close accommodation
  • Enable Care Leavers to choose whom and how they are supported and to achieve and sustain independent living
  • Engage in Education, employment or training
  • Remove any risk of falling into rent arrears
  • Address low emotional wellbeing and mental health needs identified
  • Promote emotional wellbeing and support in terms of physical health
  • Improve young person’s communication skills
  • Promote trauma informed practice
  • Address care leavers feelings of loneliness and isolation through supporting and promoting social activity, relationship building and community engagement.
  • The model will also provide support to enable care leavers to better understand their care histories and promote a life-long links approach to develop and sustain positive relationships ensuring that support continues into adulthood.
  • Have on-going and continuous relationships with children’s home’s key workers or other trusted adults chosen by care leavers

You will need to have significant experience of working with children and families in a health or social care setting, or other relevant field and a professional qualification in health, education, social care, mediation or related field or degree.
